Every Body Has A Story Testimonial Collection CONTEST OFFICIAL RULES AND REGULATIONS January 22, 2018 – February 28, 2018

To enter the Every Body Has a Story Contest (“the Contest”) simply visit http://rmalab.com/every-body-has-a-story and complete the survey for your chance to win one of two ULLA’s, one SPIRE, or two FITBITs (each, a “Prize”). Entries will be rejected if entry is not fully completed and submitted during the Contest Period, as defined below. Limit of one (1) entry per person. Contest starts at 12:00 a.m. EST on January 22, 2018 and closes on February 28, 2018 at 11:59 p.m. EST (the “Contest Period”). No purchase necessary. Void where prohibited by law.

  1. The contest sponsor is Rocky Mountain Analytical, a division of LifeLabs LP. (“Rocky Mountain Analytical” or the “Contest Sponsor”).
  2. To enter and to be eligible to win, entrant must submit a true testimonial of how either [1] a Rocky Mountain Analytical product and/or test has provided insights that positively impacted their health, AND/OR [2] submit a true testimonial of how functional medicine has positively impacted their health, where functional medicine is defined as personalized health care that treats the individual by supporting the normal healing mechanisms of the body, naturally, rather than attacking disease directly. Entrant must be a Canadian resident (excluding residents of Québec). Please note Rocky Mountain Analytical is not soliciting personal health information and does not encourage the submission of personal health information as part of this contest. A winner of a Rocky Mountain Analytical contest within the three (3) months preceding the start of this Contest (including persons designated by such winners to take ownership of prizes) and persons domiciled with a winner are not eligible to enter. Fully completing the contest, by submitting a testimonial, liking the Rocky Mountain Analytical Facebook page, following the Rocky Mountain Analytical Twitter page, and subscribing to the Rocky Mountain Analytical email newsletter, Healthy Living, will award an additional entry each (i.e., (4) chances to win) per entrant. Limit one (1) prize per entrant.
  3. Prizes - There are five (two ULLAs, one SPIRE, two FITBITs) prizes available in all provinces except Québec, and all characteristics of the Prize not expressly described herein are at the Contest Sponsor’s sole discretion.
  4. Prize must be accepted as awarded to the prize winner. Contest Sponsor reserves the right to substitute the Prize in whole or in part in the event that all or any component of the Prize is unavailable. Prize winner is solely responsible for all costs not expressly described herein.
  5. On or after March 1, 2018, at approximately 1:00 p.m. EST, a random draw for the Prize will take place at LifeLabs’ Mississauga, ON office from among all eligible entries received during the Contest Period. The first entry drawn for each province will be eligible to win a Prize. The odds of winning a Prize will depend upon the total number of eligible entries received during the Contest Period. Rocky Mountain Analytical, acting reasonably, will attempt to contact each potential Prize winner by email within two (2) business days after the draw. In the event that a potential winner cannot be contacted within 2 business days after the draw, he or she will be disqualified and an alternate potential winner will be drawn. Proof of identification must be provided upon request. In order to be declared a Prize winner, the potential winner must first correctly answer, unaided, a time limited mathematical skill testing question administered by Rocky Mountain Analytical. Before being awarded the Prize, the potential winner will be required to sign and return within the time stipulated by the Contest Sponsor, a full release of the Contest Sponsor from any and all liability of any kind arising out of the potential winner’s participation in this Contest and receipt and use of the Prize. In the event that a potential winner does not comply with all the provisions as contemplated in these Rules, Contest Sponsor shall have the right to disqualify the potential winner, and draw an alternate potential winner, and the Contest Sponsor shall be fully and completely released and discharged from any liability or responsibility in this regard. The provisions and procedures referred to above relating to selection and notification of a potential winner shall be applied, with the necessary amendments, until a qualified winner has been duly selected, but in any event, no later than March 16, 2018.
  6. By entering this Contest the entrants automatically agree to accept and abide by these Rules. All decisions of the Contest Sponsor with respect to any aspect of this Contest, including without limitation the eligibility of entries, are final and binding on all entrants in all matters as they relate to this Contest.
  7. Contest Sponsor reserves its right to cancel, terminate, modify, amend, extend or suspend the Contest including canceling any method of entry, and select a winner from previously received eligible entries. Contest Sponsor reserves its right to modify the Rules without materially affecting the terms and conditions hereof. The Contest Sponsor reserves its right in its sole discretion to disqualify any individual it finds to be tampering with the entry process or the operation of the Contest or to be acting in violation of the Rules or otherwise in a disruptive manner. Any attempts to deliberately damage the Contest or to undermine the legitimate operation of this Contest is a violation of criminal and civil laws and should such an attempt be made, the Contest Sponsor reserves its right to seek remedies and damages to the fullest extent of the law.
  8. Contest is subject to all applicable laws and regulations. By entering this Contest, each entrant consents to the collection and use of his or her personal information (i.e. information that identifies an entrant as an individual, such as home telephone number, age and home address) by the Contest Sponsor for the purposes of implementing and administering this Contest. Contest Sponsor will not sell or transmit this information to third parties .   11. Rocky Mountain Analytical is not responsible for incomplete, lost or late entries or faulty transmissions, technical computer hardware or software failures of any kind, lost or interrupted or unavailable network connections, or failed, incomplete, garbled, misrouted, scrambled or delayed computer transmission, or other errors or problems of any kind, whether mechanical, human, electronic or otherwise, which may limit or affect in any way an entrant’s ability to participate in this contest. Rocky Mountain Analytical is not responsible for any typographical or other error in the printing of the offer, administration of this contest or in the announcement of the prizes.   13. In the event of a dispute regarding who submitted an entry, the entry will be deemed to have been submitted by the authorized account holder of the email address submitted on the entry form at the time of entry. “Authorized account holder” is defined as the natural person who is assigned to an email address by an Internet service provider, online service provider or other organization responsible for assigning email addresses for the domain associated with the submitted email address.
  14. All entries from individuals found to have entered more than once will be void. In the event of a dispute over the identity of a potential winner, the potential winner may be required to provide identification sufficient to show that he/she is the person identified on the entry submission and is otherwise eligible to win.
